South Carolina & Georgia House Calls (SC & GA HC) practices a value-based healthcare model. Value-based is a better-positioned incentive that seeks to improve a patient’s overall health and outcome in a measurable way. Under this model, providers are paid based on patient outcomes rather than a fee-for-service model that ties reimbursements to the number of healthcare services delivered.
According to the Centers for Disease Control and Prevention, it is estimated that six in ten adults have at least one chronic disease, and four in ten adults have two or more chronic conditions. The management of these disease processes has become a focal point for healthcare organizations like SC & GA HC, which are looking to close care gaps and promote proactive care management.
Dr. Amber Green with SC & GA HC recently recorded a podcast for LTC University. With 21 years’ experience as a family nurse practitioner and 12 years’ experience consulting and training healthcare organizations, and practices across the US, Dr. Green believes the most significant failure of our healthcare system is that we don’t treat the whole patient.  
Dr. Green is passionate about a holistic approach to medicine and stated that “at the end of the day, we need to put our heads down on our pillows and know that we have done the best job we can do to provide the best care because we know that is what will yield the best outcome.”
